This Service Agreement and Work Authorization is for the customer identified below for the administrative assistance, inspection, diagnostic, maintenance, repair or roofing, solar, electrical, specialty, and/or related service work at the service location(s) identified below. Under this Service Agreement and Work Authorization, there explicitly is no not-to-exceed amount. For flat rates, please contact GERS offices for proposals. GERS reserves the right to decline any work or customers that do not agree with our listed terms and conditions. Filling out this agreement does not create an obligation for GERS to perform work, but acknowledges the obligation of the Customer to pay GERS for services rendered.

1. AUTHORIZATION TO PERFORM SERVICE

RATES - Check off all the below boxes as acknowledgement of the rates as publicly listed on GERS website. Additional rates and terms found on website.

I have reviewed , understand, and accept Green Earth Roofing's service pricing and terms and conditions.

Electrician Service

Electrician Service starting at $125 per man hour. Specialty diagnostics starting at $195 per man hour. Electrical Engineering Service starting at $250 per man hour.

Labor/Specialty Service

Labor Service starting at $100 per man hour. Specialty labor services, inclusive of roofing services, start at $125 per man hour. Other services/rates available .

Roofing Service

Roofing Service starting at $125 per man hour.

Administrative Service

Administrative Service starting at $100 per man hour. Design services starting at $125 per man hour. Electrical consultations starting at $200 per man hour. Structural consultations starting at $200 per man hour.

SAFETY DISCLAIMER - Depending on the nature of the service call, such as roof pitch, heavy lifting or electrical status, there may be a two person minimum for our employee's safety working on your property. If the service call requires replacement or removal of an inverter, lifting solar panels, roof access of a significant pitch or working with live power, there is a two service technician requirement.

PAYMENT DISCLAIMER- Effective August 1, 2024, all service work requires a valid credit card on file prior to the service date. In the event that a card is not on file 48 hours prior to service, the service may be cancelled at GERS's discretion. Cancellation of service within 24 hours will be subject to a cancellation fee. Please see our full page of terms and conditions regarding payments.

RIGHT TO SERVICE - Customer asserts that they have the right to designate Green Earth Roofing Solutions to perform services at the below address and that they are accepting responsibility of payment due to Green Earth Roofing Solutions for providing service
